Evaluating the Allied Health Toolkit resource
Thank you for helping us to evaluate the ELDAC Allied Health Toolkit resource. There are three sections of this survey to complete.
We will ask a couple of questions about you, about your work and how you use technology and devices as part of your everyday work. We would then like to ask you about your experience using the ELDAC Allied Health Toolkit, how you use the Toolkit to help you to provide care, your reason for using the Toolkit and your feedback on what needs to be included in the next version.
All responses are voluntary and your feedback will provide valuable information on how Allied Health professionals are using the Toolkit to better support care at the end of life provided to older people living in residential aged care facilities.
The survey should take you no longer than 15 minutes to complete.
When you have completed the survey, there is an opportunity for you to register to be involved in a 30 minute interview. In this interview, you will be invited to provide your thoughts on the Allied Health Toolkit in more detail, your feedback will help us to improve the Toolkit resource. You will receive a $25 gift card for your participation.
If you would like to participate in the interview, you will be asked at the end of the survey to submit your contact details, a researcher will then be in touch to provide you some more information about the interview process.
This study has been reviewed by the Flinders University Human Research Ethics Committee (Project Number 5860).
