This study is part of the project entitled ‘Nutrition For Ageing In Place: the NAP Project’. This project will investigate the nutritional status, nutritional intake and health outcomes of older adults who purchase support services from aged care providers using a Home Care Package.
This project aims to explore the perspectives of case managers and the services provided in the context of Home Care Packages. This survey will collect information about you, your experience as a case manager working with Home Care Package providers, and your views on food and nutrition services. Please follow the instructions for each question and read and answer each question carefully.
To participate in this survey, you must be aged 18 years or older, be competent in English, and be currently employed as a case manager for a Home Care Package provider.
Your individual responses will remain confidential and will not be shared with anyone. Please do not write your name anywhere on this survey. All information provided will be treated in strict confidence and will not be made available to any other source.
This survey should take approximately 15-20 minutes to complete. By participating in this survey, you are providing your consent to be involved in this research study.
