  SCHOOL QUESTIONNAIRE  
 
 
Teachers could you please read out the following statement to students:
When you begin the questionnaire you will be asked to provide some details about you and the school. You will be asked about:
  • The School Code – For our school, it is (initials of the school). So write the school’s initials there.
  • The Class ID – in that box write (whatever the class ID is)
  • Student ID – in this box you are to write the initials of your name – so the first letter of your first name, your middle name, and your last name. THEN, you are to write the year of birth. So if someone was born in 2003 they would put 2003 after their initials.
This information is REALLY important. Without this information – which you must write exactly the same each time you do the questionnaire, the researchers won’t be able to tell if what has been done in class has worked the way it was intended to.
 
 
This is a questionnaire about life at school this year for you.

The questions are mainly to help you to describe your thoughts and feelings about school and what goes on between students at your school. The questions focus on your happiness & wellbeing and connection to school and on the issue of your relationships with other students. 

If you do not want to answer any questions you are free not to. Your answers are confidential so no one will know what you have written. THANK YOU FOR YOUR HELP.
