      • I have read and understood the information about the research, and I understand I am being asked to provide informed consent to participate in this research study.
     • I understand that I can contact the research team if I have further questions about this research study.
     • I understand that whether my child participates or not, or withdraws after participating, will have no effect on any service that is being provided to them by the school or university.
     • I understand that, with one exception, only the researchers listed on this project will have access to my child’s personal information. The exception is regarding the storage of my child's full name and their responses on the Reading Doctor software on the Reading Doctor web-server. I provide consent for this data to be stored, understanding that it will only be accessible via password and user login. Access to this data will be restricted to members of the research team who will be provided with the necessary login credentials by Reading Doctor.
     • I understand that a large-scale deidentified data set will be developed. Information in this data set will be shared with a statistical consultant for the purpose of analysis.
     • I understand that the deidentified data set may be shared with other researchers, to promote transparency, accountability and/or enable further investigation and outcomes of this valuable research.
     • I understand that my involvement is confidential, and that the information collected may be published.
     • I understand that my child will not be identified in any research products.
     • I understand that my child may not directly benefit from taking part in this research.
• I agree/ disagree to being contacted about another related research project seeking to explore the perspectives and experiences of children and parents who have used Reading Doctor® in this study.
Thank you for providing your consent and confirming your child’s assent to participate in this research project. The researchers are seeking a small group of children who live in the greater Adelaide metropolitan area and are willing to complete an in-person assessment with the researcher in addition to the usual online assessment. This will assist the researchers in understanding the agreement between online and face-to-face assessment.
